Mental Health Community Partners Program

The Mental Health Community Partners (MHCP) Program supports the recovery of youth  12+ and adults managing behavioral health conditions in their transition from unwell to well, and successfully connecting to community supports. Individuals in recovery participate in the 90-day program with their family or chosen supporters as they prepare for a new stage of life in the community. Individuals and their chosen supporters are supported by a certified Mental Health Community Partner, who guides the family in achieving whole-family health and wellbeing, and preventing re-hospitalization.

The Mental Health Community Partners Program is available across New York State. We work with individuals transitioning from any type of treatment setting programs, community housing, correctional facilities, etc.

The Mental Health Community Partners Program is brought to you by the Mental Health Association in New York State, Inc. (MHANYS) in partnership with the New York State Office of Mental Health (OMH).
