Mental Health Community Partners Program

The Mental Health Community Partners (MHCP) Program supports the recovery of adults with serious mental illness and youth aged 12+ with serious emotional disturbance as they transition from any treatment level or setting to the community. Individuals in recovery participate in the 90-day program with their family or chosen supporters as they prepare for a new stage of life in the community. Individuals and their chosen supporters are supported by a certified Mental Health Community Partner, who guides the family in achieving whole-family health and wellbeing, and preventing re-hospitalization.

The Mental Health Community Partners Program is available across New York State. We work with individuals transitioning from any type of treatment setting or level including psychiatric hospitals, residential facilities, rehabilitation programs, community housing, correctional facilities, etc.

The Mental Health Community Partners Program is brought to you by the Mental Health Association in New York State, Inc. (MHANYS) in partnership with the New York State Office of Mental Health (OMH).
