Mental Health Community Partners for Individuals in Recovery and Their Families

When someone in recovery transitions out of a treatment setting, family members or chosen supporters can help provide effective and meaningful support. Mental Health Community Partners are here to help.

Your Certified Mental Health Community Partner Will:

  • Meet with you weekly over 90 days
  • Guide you and your family in learning about the cycle of recovery and whole-family health
  • Provide follow-up support to prevent re-hospitalization and ER visits
  • Support successful transition to the community

This program is offered in the format that works best for you and your family: in-person, online, or hybrid.

Is MHCP Right for Me?

The Mental Health Community Partners Program may be a good fit for you if:
  • You or a loved one identifies as an individual in recovery managing a behavioral health condition

  • You or a loved one are transitioning from unwell to well, and seeking supportive connections in your community

  • You or your loved one in recovery are seeking to involve your family or chosen supporters as participants in the program
    Individuals who do not have identified family members or chosen supporters to participate in the program may still participate as an individual, and receive support from the Mental Health Community Partner

  • You or your loved one in recovery is over 12+ years of age

  • You live in New York State

* Upon completion of the Mental Health Community Partners Program, participating individuals and families are eligible to enroll in the MHANYS CarePath™ Program for an additional 90 days of support from a certified CarePath™ Coach.
