This program is offered in the format that works best for you and your family: in-person, online, or hybrid.
You or a loved one identifies as an individual in recovery with a diagnosed mental health disorder
You or your loved one in recovery are seeking to involve your family or chosen supporters as participants in the program
Individuals who do not have identified family members or chosen supporters to participate in the program may still participate as an individual, and receive support from the Mental Health Community Partner
You or your loved one in recovery is over 12+ years of age

* Upon completion of the Mental Health Community Partners Program, participating individuals and families are eligible to enroll in the MHANYS CarePath™ Program for an additional 90 days of support from a certified CarePath™ Coach.
